Ordering Options:

                        We accept cash, money orders, checks, debit cards, and Bridge Cards (food stamps). Ordering dates:

Thurs. 12/3 Otisville (Forest Twp. Senior Center) - 11:00AM to NOON; Fri. 12/4- 2:00 PM - 6:00 PM; Sat. 12/5- 9:00 AM - 11:00 AM  

There are 3 ways to place an order: 

1) In Person:  Come to Otisville Assembly of God  (OAG) or the Forest Twp. Senior Center on the designated dates and times.  Food Stamp orders must come to OAG to use this form of ordering as we need your signature on a voucher.

2) Mail-in ordering:  Visit our web page at www.otisvilleag.org and click on the Angel Food Ministry tab then menu/on-line ordering, print the order form. Or, pickup a menu at OAG and cut off the mail-in order form and mail in your order.  Please ensure we receive your mail-in order by the indicated cut-off date.

3)  Order on line:  Follow instructions on this web site.

Please remember to bring a large box or cooler for the items in the Signature Box.
